Mike the Chauffeur
I read a post from travelprincess about a chauffeur in Prague named Mike. I found his website and have e-mailed him twice in the last week but have not gotten a response. Does anyone know if he is still in business? Or know of someone else (trustworthy) I can contact to take 2 of us from Prague to Munich? We need to catch our flight back to SFO from Munich at 3:50 pm (3/28)and would like to spend as much time in Prague as possible so think this might be the best way to do that.

"...think this might be the best way to do that." Well, I don't think you'll get much agreement on that. That's a 405km drive that takes four hours.
Have you considered taking a train the previous evening? You aren't going to get much sightseeing done on the 28th anyway, and there's a direct 17:16 that arrives at Munich main rail station (Hauptbahnhof) at 23:26 - for €51,30.
Next day, it's a 40-minute ride from the same station to the airport - for €8,40.

p.s. Traveling from another city on flight day is a gamble. I always spend the last night somewhere that has at least two ways to get to the airport on time.

NOT "the best way to do that"," at all, IMO. I think it's courting disaster to be that far away from your airport the morning of your departure, and what are you gaining? Another hour in Prague?
I'd take the train the night before.

I think it's a bad idea, also. The train route isn't always that direct between the two points, I know, because I was considering it last year and a lot of trains require changes. However, there are usually one or two a day without changes, so I would take that one at 5:16 pm myself. I would'nt count on someone being able to drive that route in four hours in order to make a flight. That's an average of 100 km per hour, and just getting out of Prague if you are in a central area can take a while.
If you really want to do this, though, I'd ask your hotel if they know of someone reliable who would be willing to do that. I certainly wouldn't count on someone to make a flight who has a website and won't answer his email.

Perhaps more information about our travel plans would be helpful. We are traveling with 6 others from Vienna, to Salzburg and then Prague and dispersing from there to various other places. I have two round trip airline tickets, from SFO to Munich/Munich to Vienna for the congress - then Vienna to Munich and Munich to SFO home on 3/28. I'm trying to avoid having to go back to Vienna on 3/27 and spend an extra night with my friends in Prague. It seems as if, from your responses, that it would be best to go back to Vienna and start the journey home from there. Don't have an 'open-jaw' ticket so seems as if I'm stuck with that plan. Thank you all for your advise.

If you're flying home from Munich, I don't see much point in going back to Vienna - unless I'm missing something. It would only cost more and risk missing the plane.
I'd stick with the 17:16 train from Prague to Munich Hbf on the 27th, overnight in Munich, and take the S-Bahn (commuter train) to the airport. That would even allow a morning to see a few Munich sights, if that interests you.
