Best car/shuttle service from Paris to CDG

If we change some reservations and stay in Paris the last night of our visit, but need to be at the airport really early, we will need a car/shuttle service. We will want to be at CDG by about 5 am (so can't rely on RER or such or even some "normal" hotel/airport shuttles) so will have to arrange for pickup very very early. Which car/shuttle service would you recommend (just one way, from hotel in Paris to CDG)?

These all seem the same price and none seem to require advance payment. We'd prefer to have door-to-door personal pickup, not shared "shuttle" that even at 4ish am might be making stops.
