For Machu Picchu and Cusco I went with http://mythicaltrailsperu.com/index.html they were frienly, they took care of me all the time and gave me a cellphone to contact them anytime. The Hotels and restaurants that they provided were good (save and clean).

You questions are broad enough that I think a good guidebook would serve you well. This is the Fodor's site, so I'll recommend the Fodor's guide to Peru. I think it's exceptionally well done. And I'm not just saying that because I've worked on every edition of the book so far.

Our favorite restauarant in Cusco was Chicha - don't miss it! As far as hotels in Cusco, we stayed at the Rumi Punko, which was more than we wanted to spend but spotless with great staff. We didn't like the sounds of reviews for the less expensive level of hotel/hostel.
