The very best hotel...

...in always in the eye of the beholder.

Different things are important to different people.

Tiff said it best.

That being said, if money was no object, I'd certainly look into ...

Rome -- http://www.hotelhassler.com/

Florence -- http://www.lungarnohotels.com/

I've stayed at neither, but I have heard that rooms can be small and even dingy at the Hassler Villa Medici in Rome. THE in luxury hotel in Rome at the moment is the Hotel de Russie:

The location is excellent, and I've seen only praise for the service and the ambience.

I don't know if you can call any hotel "the best" because it's a matter of opinion, but The Lungarno in Florence, right on the Arno, just off the Ponte Vecchio, has beautiful rooms, excellent service, unbeatable location.

I stayed there on my first trip to Florence. We loved it so much that a planned 3-day stay turned into 8.
