To clarify the above, I somehow lost my CDC card (I know!!!!) so I obtained an FL Shots Immunization History from the FL Department of Health that shows I received both doses of Moderna. There are no lot numbers on that. Of course, I'm very worried that France will balk at accepting this as proof and can't get answers from anyone. And, we have one train segment in our trip towards the end, so hopefully I can find a pharmacy to help with the bar code before that. Just so much to worry about.

Barefiootbeach, you've likely already considered this, but in case not....
For a copy, can you return to where you received your vaccinations? That place (clinic, pharmacy, etc)should have its own record that shows your vaccine's lot numbers and expiration dates.

Russ, first of all, I'm so grateful for your post that alerted me to the complexities of the pass and secondly, that Lot #'s existed!
CaliNurse, I re-contacted the FL Health Dept about the lack of lot #'s on my print out...which also surprised her. She was able to send me a different printout that did have the lot numbers listed.
As I still share your belief that anything out of the ordinary (like my printout instead of a CDC card) is likely to be rejected by bureaucracies, and with the extreme rise in cases of the Delta variant that might close borders, and the possibility that we vaccinated could be symptomatic yet test positive, it was with great disappointment that I cancelled my flights, all the inns and restaurant reservations I made. Au Revoir, Normandy & Brittany...perhaps in the spring. One more trip down the drain. I know this is a first world problem, but still.....

At cafés, restaurants, museums and cinemas, not to mention trains, you will not be dealing with "bureaucracies" -- you will be dealing with completely untrained people who never dreamed that they would suddenly have to check health passes in addition to their normal work. This is sure to make enforcement very spotty with some people very easy to convince if your document is a bit dubious and a few who will revel in the sudden power that they have to lord it over you.

. . . . but not the ones with the EU pass sanitaire (and they are the vast majority of the foreign tourists in Paris for the moment -- mostly Germans and Benelux)..

Astuce : au moment où le professionnel de santé vous demandera votre code postal, dites-lui de saisir « 99 » puis de sélectionner Etats-Unis dans la liste des pays. Il lui faudra sélectionner la rubrique « Autres lots » dans le menu déroulant au moment de saisir vos numéros de lot. En moins de 5 minutes, il sera capable d’imprimer une feuille avec le QR Code.

I received an automated email this morning saying they'd process it as soon as possible and for me to be patient. Although it says that it applies to people traveling prior to August 15, other Americans departing after the 15th have had success obtaining a QR code after submitting their documents on this site. I'm keeping my fingers crossed and I'll post here either way as to my success - or not.

We sent my husbands info in yesterday afternoon and he got a return that it was received. Sent mine last night and still have not gotten any return email that it was rec'd.
