I think it's easier to take the train from Rome to Salerno, and then catch a boat from there to Positano. At least that's the advice I received and it worked out perfectl for us. It's also beautiful to see the Amalfi Coast from the water.
The boat dock is about 10 minute walk from the train station in Salerno-- really easy.

I think it is easier to simply take the Circumvesuviana (light rail train) to Sorrento, then a bus to Positano. The ferry connection in Positano can be a bit rough -- you transfer between the ferry and a small landing craft -- and if you have problems walking it might prove difficult.

Hi Pipps,
I would take the shuttle express from FCO to Rome TE (about 23E). Cab will be about 45E.
From Rome, I would take the train to Salerno. The ferry docks (Piazza Concordia) are about 4 blks from the train station.
You can take the ferry from Salerno to Positano, or get off in Amalfi and take the sita bus.
Going by your route, you have to navigate from the train station to the docks in Naples. Take the no. 1 tram or a cab.

I would reserve a car service from the Rome airport to your hotel; the one I use charges 40 euro.
