flying into to Rome and flying out of Milan: Is this too crazy?
Please let me think what you think about this itinerary:
Rome: 2 nights
Sorrento: 2 nights
Positano: 2 nights
Siena: 3 nights
Porofino/Genoa: 3 nights
driving to Milan in the morning to fly out or
should we drop off the car in Genoa area and ride the train to Milan?

I would take the train to Milan - only 1 1/2 hr on the IC, unless you are getting a weekly rate on the car and it would cost more than 35E pp.
Why are you splitting the AC between Sorrento and Positano? Why not stay in one and visit the other?

I agree with Ira on the Sorrento-Positano thing and I would choose Sorrento. You will lose a lot of time traveling between the Amalfi Coast area and Siena and I would drop Genoa in favor of more time at Portofino unless you need to be closer to Milan for an early flight. Have fun.

I'd probably keep the car and drive to Milan airport. It doesn't make much sense to me to take a train to Milan, then have to transfer to the airport, since you aren't planning on staying in Milan at all. Easier to drive right to the airport with your luggage and turn in the car there.

Rome 3 nights. You need it
Sorrento 3 nights, and visit Positano and Amalfi Drive.
Siena and vacinity 3 nights and
Santa Marguarita Ligure 3 nights, visit Portifino, Camogli, and Rapallo. Drive from here to Genoa, take the train to Milano.

I concur with most but . . . why I think you are trying to cover too much ground. Consider canceling the Portofino leg entirely or try: Rome 4 nights (unless you have been there before and didn't want to 'do' it again, you are cutting short a magical city). Positano 3, Siena 3, Portofino 2. Just my thoughts.

I will look into just staying in Sorrento and doing a trip to Positano
and Capri.....
I was hoping to stay in Positano but looks like everything is getting booked up.
This is my 2nd time in Rome so I don't need to do the whole tourist thing again.
