I must reenforce: careful crossing streets, drivers stop for no one that don't use the stop signs, that's how Paul Mccarthy's wife lost her leg.

Stay clear of Princess Michael of Kent, especially if you are a minority who speaks loudly. (You will be told to go back to the colonies, darling.)
Don't tell anyone you are American. They will think you are daft.

Don't stare at people's teeth. The English have the absolute WORST teeth imaginable. (Thankfully, my parents sent me to the orthodontist when I was 12 years old.)

You will be reminded to "Mind the gap." Don't worry about that one.
Crossing streets is funny. Painted on the street at many many intersections are the words, "Look right" or "Look left" just so you know where traffic is. It sounds silly but it may save your life.
St. Paul's is swathed in scaffolding right now so plan to buy photos from a gift shop. Do attend the Choral Evensong there though. It's magnificent and you can attend for free . . . as opposed to paying several pounds to look at a lot of scaffolding earlier in the day!
In Hyde Park at the southeast end of the Serpentine, a lady with bags of bird food arrives at sunset to feed the birds. You will see herons, egrets, swans, geese and various other waterfowl swimming down to greet her. It's lots of fun.
If you want tea and don't want to spend a fortune or get terribly dressed up, go to the Orangery at Kensington Palace. They serve until at least 5:00 PM.

In case you haven't seen it, I called up an old thread on 100 Great Things to do in London. I think you will find this list very helpful.
Also, please note the posters on crossing the street and Janis' post on standing on the right on the escalators in the tube stations. People in London are, it seems, always in a hurry. You definitely do not want to get run over!
I have been to London 7 times and the excitement of going to my favorite city has not worn off. Don't try to do everything in your first trip. You can't. I feel that I have barely scratched the surface and am already planning my next visit.
