What is the train like - brillant,
what is Tokyo like - brillant,
Worth the extra money - yes.
I've been to Japan many times, and will continue to go back, the bullet train is very comfortable. However, in some of the larger stations there can be a fair amount of walking.
Will your cruise end in Yokohama or a port nearby Tokyo, or do you just have a day in port?

If you end in Osaka, you may want to consider going to Kyoto, the historic sights there are worth seeing. From Kyoto I'd then travel to Tokyo, assuming you haven't been to Japan.
If you know which sights you want to visit, maybe fodorites can advise how much walking will be involved.

Imho, yes it is. Japan is a jewel in Asia waiting for you to explore. Please do some research and come back with more specific questions.
Aloha!

I think Japan in general and Tokyo in particular is a fantastic place to visit. However, the OP's mention of "limited mobility" raises a flag for me.
Getting around in Tokyo, or getting between cities, always requires a fair bit of walking, and often includes stairs. That also goes for many of the sites in a city such as Kyoto.
