Lunch is VERY casual everywhere. We had dinner at Mozaic and it was a tad more dressed than other places we went to eg the night we went no-one was in shorts. Do try the Balinese wine, Hatten while there - their rose is very reasonable and a great accompaniment to local cuisine. They won an award for their white last year in Luxembourg.

As others have mentioned Bali is casual everywhere. Because Mosaic is "romantic" in feel, people do tend to dress up, but it's not a dress code per se.
They have a prix fix (sorry I can't spell it) menu that includes either 3 or 5 courses. Imported wines are expensive but very good. I don't drink but my husband likes to experiment with good wines, so we kind of average either out. A meal for the two of us three courses, three or four glasses of imported wine would be around $US100.
