totally flat seats/beds

My choice would be Swiss. MIA-ZRH is flat seat in business (and of course first), then a short flight with excellent connection times in both directions to Rome. The extra 70-80 minutes drive from PBI would be absolutely worth it to me because of the superior product and good connections.
If you want to go out of PBI, you can take Delta and change in Atlanta, or US Airways changing in Philadelphia. Dunno if they offer flat seats.

re BA: Oh, yeah, them too. Flat beds as good or better than Swiss, service maybe not so much, LHR a somewhat more aggravating airport than ZRH, but not as bad as some would claim (as long as you don't need to change terminals).

There are more options than those listed above.
From FLL: You can fly Air Canada connecting through Toronto. The flight to Canada will be in standard short-haul first class seats, which are better than the short-haul business class seats on European carriers. The long-haul portion will be in flat beds. Note, however, that the arrangement of their flat beds means it can be a bit tough to interact with a traveling partner.
From FLL or PBI: Depending upon your timing, Continental does fly a 777 with flat beds from Newark to Rome during the summer. During the winter, this looks to go to a 767, which will not have flat seats.
Delta also has their 767-400 scheduled on the ATL-FCO route, and this looks to run well into the schedule. This offers pretty much the same seat as that offered by Swiss. You could fly from FLL or PBI.
Bear in mind that both Delta and Continental have planes without flat beds, so an equipment change could leave you without a flat bed. Swiss, too.
From MIA: In addition to Swiss or BA, you can also usually book a ticket with Virgin Atlantic, connecting to another airline (in this case, BA or Alitalia).
You can also fly on Delta from MIA to LHR and connect to Alitalia there. Again, this will be on a 767-400 for the long-haul portions.
If I had to give an opinion... I'd probably be most inclined to go with Delta, via Atlanta, ASSUMING you could confirm that the 767-400 is the plane scheduled. This minimizes your short-haul flight and lets you fly from FLL or PBI. Air Canada or Continental (again, assuming you can confirm that a flat-bed plane is scheduled for CO) would be decent choices too, though with longer short-haul segments.
If I were flying out of MIA, the choice is a bit more up in the air. Delta and Swiss offer the best seating for couples traveling together. BA and Virgin are both nice, with very good ground facilities at Heathrow. Delta also shares the excellent SkyTeam lounge at Heathrow with Alitalia and has a new lounge at MIA.
For in-air service/food, I would say Swiss is probably the best, followed closely by Delta. BA is definitely a step behind. Don't have much recent personal experience with Virgin, but everything I've read suggests that service can be a bit erratic and that the in-flight food is poor.
