Cathay Pacific or Japan Airlines will get you there. CX with a connection in HKG or Japan through Tokyo.

CX flies from JFK, LAX or SFO
Japan flies from JFK, ORD, LAX and maybe SFO(not sure)

The ticket won't be that much more if it's done from your neares major airport.

American Airlines are part of the OneWorld Alliance so they can get you to the gateway city for few extra $.

Try all three websites to get the best deal

www.aa.com
www.japanair.com
www.cx.com

Where's "a City in the United States?"

If that city is Honolulu, you can fly Continental via Guam twice a week.

It depends where you are flying from.

From East Coast Singapore Airlines, non stop to Singapore and connect there easily through Singapore onwatds to Bali, about 2hrs 30 Singapore to Bali.
You can also Thai non stop to Bangkok and then continue to Bali, but Singapore Airlines has more flights into Bali.

From West Coast as others have suggested Cathay etc.
