Seriously, you need to provide more information if you want anyone to offer any worthwhile advice. Where are you starting out from, for example? If you enter your origin and destination into a booking engine like Orbitz, there really isn't much mystery about possible routing, schedules and fares, in general terms.

The most comfortable way for a 90yr old might probably be a cruise ship. I'm being serious! Antwerp has recently become a port of call for many cruiseships. The ships dock on the river Schelde in the middle of the historic centre.

To answer the question, the best way would be to fly to Amsterdam (non-stops are available from many US cities). From the Amsterdam airport (Schiphol, pronounced "ski pole", you can take a train directly to Antwerp.

Just got back from Amsterdam, with a day trip by train to Brussels, which took us right thru Antwerp. Great, easy trip. Great way to see the countryside.
Are you visiting relatives and are anxious to get to Antwerp? Otherwise, this 60 yr. old needed to rest after the flight so I'm glad my hotel was in Amsterdam!

I agree with <b>Tulips</b> suggestion. Brussels is closer to Antwerp. There is a bus that runs every hour to central Antwerp. Cost is ~8EUR and it takes ~45 minutes.
Train from Amsterdam takes ~2 hours.
but, it all depends where you flying from. If you could fly to AMS on a non-stop from your city, a more likely possibility, then flight+train may make more sense. On the other hand, if you could take a non-stop to BRU, then fly+bus is the way to go. There is also a train from Brussels airport to Antwerp but it involves a transfer in Brussels.
The one non-stop to Brussels from US that I know of is on American from Chicago. I'm sure there are more, probably from NYC on other airlines, but Amsterdam will probably have more non-stops from US as it is a hub for Northwest/KLM airlines.
If you let us know where you are flying from, we could be more helpful.

If you look on www.brusselsairport.be (click on English first) you can get a full list of destinations from Brussels.
There's also an airport in Antwerp, but it's very small, and only European destinations (5 flights a day to London City).

Thank you so much for all the suggestions!
We will be leaving from the midwest (Kansas City). I am considering the advice flying to either Amsterdam or Brussels and staying overnight for a rest before we travel on to Antwerp. We are taking a riverboat cruise from Antwerp to Amsterdam to see the tulips, so perhaps flying into Brussels a couple of days earlier will be nice.

Fly into Brussels and from Brussels airport take either a direct bus to downtown Antwerp (one every hour for €8) or book a limo that will get you there in half an hour for around €70(www.antwerplimo.be)
If you'd rather stay in Brussels city for while first, there are trains from downtown Bruxelles to Antwerp every 30 minutes.
