We used LimoServiceRome.com to take us from our hotel to the airport although they can also pick you up at the airport and take you to your hotel.
It was $45 euros for my husband and I and Andrea was right on time when he picked us up. I know others on this board have used this service and have been pleased.
The rates are listed on the website depending on how many people you are traveling with.
You can also take the train although I don't have the details on which train to take. It seemed too much of a hassle for us.

I would second a private car service. I have used a taxi directly to our hotel from the airport and the train and taxi combination from the airport. I also used the suggestions found here for avoiding being scammed. I thought we were being very cautious both times and still we were scammed. The train would be ok if you had a hotel close to the station.

I tried twice yesterday to reply but with the problems nothing worked so I am trying again, third time lucky.
We were there less than two weeks ago and like the others I would recommend a limo. It depends on how many are travelling, if it is just yourself then maybe the train and taxi would be more economical, not more convenient.
We drove from Stresa to Rome airport and promised ourselves a limo after such a long day. Anyway the Avis rent a car chap said to catch the train it was so easy. We were swayed, unfortunately, and took the train. It was a fairly long walk to the station with heavy bags, the train was crowded, stop, start movement when we finally got going. We got a taxi at the terminii and I made sure it had a meter etc. However, he managed to obscure the meter during the trip with the front middle seat of the taxi and hey presto by the time we arrived at the hotel all of 10 - 12mins journey the meter now read 27E. A total rip off. It doesn't matter how old you are or how many times you travel you still get ripped off.
It should have been about 15E. So it ended up costing us as much as a limo. The train fare is 9.5E each. Needless to say we caught a limo back to the airport from the hotel when we left. A lovey Merc, door to door comfort.
So if you take the train and taxi, be warned, keep the meter in view AT ALL TIMES.

I took a shuttle pre-arranged by the hotel for 12 Euro. I believe the shuttle was through "Hotel Reservation" and it had to be arranged in advance by the hotel and I paid for it at the hotel. I found this to be convenient and economical - I was dropped off at the hotel and didn't have to drag my bag to the metro (??) and then through the train station to the FCO direct train. The 2 Euro extra was worth saving all that hassle.
