It depends on what you are looking for. Are you a couple, a group of friends, or a family? Here's what we enjoy when we go down there.
Family-type:
1) Tom & Jerry's--not the best food, but pretty good. Very kid-friendly.
2) Louie's Backyard--has a pretty good seafood buffet, but a little pricey; good music and fun atmosphere.
3) Roma Dolce--awesome gelato.
Good food, but still kid-friendly:
1) Blackbeards--good seafood, but a little overrated IMHO
2) Padre Island Brewing Company--good beer, good food, and really good ribs!
3) Rovans--good breakfasts
4) Shrimp Haus--awesome if you do all-u-can-eat shrimp
5) Bigo's--we liked it for Mexican
In Port Isabel (just on the other side of the bridge):
1) Beefeaters--all-u-can-eat meat
2) Lighthouse Ice Cream (I think that's what it is called. It is right off the bridge across from Beefeaters)--homemade ice cream, really cool flavors.
A few we didn't like:
1) Jesse's Cantina--way too greasy for us
2) A 50-s style bar and grill on the island...further north on the island and on the western side--can't remember the name--but not that good and way too expensive for what you get.

There is an Italian restaurant just over the bridge in Port Isabel. It's on the right hand side, can't remember the name right now...
But if it's still there, we enjoyed our meal there very much.
We did not care for Blackbeard's. Overpriced and underwhelmed.

Heading down there next week-- taking all your suggestions with me. Memejw -- I know the Italian place you are talking about; there's a karoke bar on one side, as I recall. Beach here I come!
