How/when does one tip the Concierge?

How/when does one tip the Concierge?
This may seem like a strange question but I've never felt the need to tip a concierge. I've asked where to get a breakfast in the morning or where a place is located but I've never tipped for that advice. What exactly deserves a tip, how much and how do you give it to them? Do you just say "thank you, I appreciate it" and hand them money? I would really like to know because I do tip and I want to do the right thing. Just not sure on this issue. Any advice would be appreciated!

We typically use the concierge to make restaurant reservations prior to our arrival and obtain theatre tickets. We tend to give between $10 (simple reservations) and $25 (theatre tickets). We put the money in a hotel envelope with the concierge's name who assisted us (typically the confirmation emailed or faxed prior to arrival gives their name).

I only tip if they get tickets, or manage to get you into a restaurant/bar you couldn't otherwise get into on your own. I've never tipped for directions!
We've generally tipped after the fact (When getting difficult reservations, you usually find out via a message as they don't usually get them right away). When we pass the desk on our way out we just hand them the money and say thank you.
