Highlands has more of a small town feel...more restaurants, shopping, etc.
Cashiers is smaller and is mainly the High Hampton Inn.
In both areas are nice B&B's, golf, hiking, waterfalls.
Neither area is known for their nightlife.
By the way, dmlove, we're talking about the North Carolina mountains. Many Floridians have second homes there.

There is not much in Cashiers--pronounced Cashers. Highlands is much more attractive with more options on lodging and restaurants. The Old Edwards Inn is gorgeous but not inexpensive. They have a beautiful spa and rooms are luxurious with Frette bed linens and towels. Very pretty place.

I would say if you want to be able to walk to several things without driving--then stay in Highlands. There are places all the way from Highlands to Sapphire Valley. Just depends upon what you looking for!
You can stay at the High Hampton and have almost everything provided for you in Cashiers.
