car rental info?
When I rent a car, I am always confused about whether I need the additional insurance, collision damage waiver, etc. If I put the rental on my american express card, or on another charge card, can I safely refuse these charges?

You should also contact your insurance company and find out what they cover. I've had a couple rental agency people tell me that my insurance wouldn't cover the rental in that state, even though my insurance agent said I was fully covered. Some rental companies are well-known for trying to scare customers into buying unneeded extra insurance.

I agree with contacting your insurance company.
I worked for an insurance company and all of the coverages/amounts that you had on your privately owned vehicle applied to your rental car, but it's possible that this could differ either state to state or by insurance company.
