My understanding is that they're very rare here in the States because of liquor laws. The only resort that approaches all-inclusive in Hawaii is Kona Village on the Big Island. It includes meals and many activities, but not drinks and it's certainly not a "deal" like Caribbean AIs (rooms start at $600 per night).

Club Med has a resort near Port St Lucie, FL. It is a family resort called the Sandpiper. You can get more info on Club Med's website.
MerryTravel is right - there are very few AIs in the US like you would find in the Caribbean. Whether it's liquor laws, or market forces - who knows. All I know is that we did significant looking last year and the Sandpiper was the only one I turned up.
