Try this: http://www.parisvisite.tm.fr/musmon/welcome.html.

I know Sainte Chappelle is covered, because I went there last week on the museum pass; without looking at the list on the website, I don't know about the others.

The Museum pass is worth it even if it doesn't include everything you want to see because it allows you to cut the lines, which in the summer can be quite long. We walked right into the Louvre, d'Orsay and Sainte-chappelle when the lines were around the block!

Even though the Garnier Opera House is not included on the museum pass, you shouldn't miss it.

The claim to "include" a musuem is a bit of a stretch. I was in two museums in the first of May that had become free museums since Dec. 2001.

On was the Musée Cognacq-Jay - Musée du XVIIIe siècle de la Ville de Paris. The other was Musée Carnavalet - Histoire de Paris.

Others may be the same so if you are number crunching, you might take that into account.

To clear up some confusion, the museums David mentioned are/were not part of the group covered by the museum pass. They are city museums, I believe, and, as such, are now free.
