I still get recurring whiffs of cigarette smoke on the "non-smoking" train cars. So you can imagine what the "smoking" cars are like.

Until next year, smoking is permitted ONLY in designated Intercity (IC) railcars on most Italian Intercity trains. There is usually ONE such railcar per class of service. If you want to smoke, you should request a seat in a smoking railcar of the Intercity train.

Smoking is NOT allowed on ANY OTHER type of train in Italy, including:

Eurostar Italia
Interregionale
Regionale
Diretto
Espresso

Smoking is never allowed in restaurant or restaurant/bar cars.

Most IC trains in Switzerland have completely separate non-smoking cars, while some other routes have the non-smoking section separated by a glass partition.
