Southern Italy
I am staying in Positano, leaving on Semptember 23rd to catch an international flight at Fiumicino at 14:30. So, I need to be at the airport at 11:30. What is the best way to do that?

Be realistic. Return to Rome on the 22nd and save yourself the stress of all that worry. Otherwise you will not enjoy your last days. And if something unpredictable happens the morning of the 23rd and you miss your flight, then what will you do?

If you can afford it, a private transfer Positano to FCO will cost you around 500 euro. There is no public transportation that will get you to FCO by 11.30am. You will have to stay the night before in Rome as advised above.

Leaving for FCO from Positano on the same day you are catching an international flight is NOT a cogent strategy. IMHO, it is a really bad idea. Say in Roma the night before you depart for peace of mind, alone.

If you MUST spend the night of the 22nd in Positano I think the only possible solution is for you to find a rental car there that you can drop off at the FCO airport, but only if you leave Positano very early in the morning.
