Okay, here's my third draft. What about taking a train from Rome to Siena and picking up the rental car in Siena? I like the idea of Lake Como for the first night, but I'm not sure about spending more time on a train between Milan to Lake Como, then Lake Como to Venice. I thought about moving Lake Como to the last full day before our flight home, but I still don't want to spend our last full day of our trip traveling. So here's the current version of our itinerary. The rental car is still an issue. Pick up in Rome? or Siena? Drop off in Florence?
Day 1: Arrive in Milan, take next train to Venice
Day 2: Venice
Day 3: Venice
Day 4: Leave Venice, train to Rome
Day 5: Rome
Day 6: Rome
Day 7: Rome
Day 8: Rome
Day 9: Leave Rome for Tuscany via rental car (pickup in Rome or take train to Orvieto or Siena to pick up the car)
Day 10: Tuscany
Day 11: Tuscany
Day 12: Tuscany
Day 13: Tuscany
Day 14 Sunday: Leave Tuscany, drive to Florence to drop off car at airport, take bus into the city? then stay overnight
Day 15: Take train from Florence to Milan n the morning, 4 pm flight in Milan
Day 1: Arrive in Milan, take next train to Venice
Day 2: Venice
Day 3: Venice
Day 4: Leave Venice, train to Rome
Day 5: Rome
Day 6: Rome
Day 7: Rome
Day 8: Rome
Day 9: Leave Rome for Tuscany via rental car (pickup in Rome or take train to Orvieto or Siena to pick up the car)
Day 10: Tuscany
Day 11: Tuscany
Day 12: Tuscany
Day 13: Tuscany
Day 14 Sunday: Leave Tuscany, drive to Florence to drop off car at airport, take bus into the city? then stay overnight
Day 15: Take train from Florence to Milan n the morning, 4 pm flight in Milan

"DebitNM: If I wanted to sleep, would seats 48 and 49 not be a good idea then? "
I found them to be fine going to Milan; or maybe it was the Ambien
For the flight to JFK - since you leave at 4;10 pm or there abouts; you won't really be all that tired until almost the end of the flight. I tried to sleep [1/2 ambien] but I just couldn't. Part of it was all the noise in the galley which is very close to those seats. Lots of people hanging out there; chatting with FA's or each other.
I just looked- you may be flying a A380. We did not, we flew Boeing 777-300ER; so all my input on seats may be moot. You should be able to see the euipment on your reservation.
I’ll probably try to sleep both trips. Also, when can you book your seats? When I booked my flight, it jst asked me for my preference of window/aisle,m not the actual number.
I can't recall when but not long after. You have to log onto the website and use "Manage myBooking" on right side of page.

I like your revised itinerary, if you're good with the risk of getting back to Milan the day you leave!
If I were in your shoes, I'd probably lean toward taking the train to Siena, store my luggage, see what I wanted to see there and then pick up the car from there. I haven't taken the train there, though, so I'm not sure how easy it is to get from the station to the main sights.
We were just in Florence from Sunday afternoon to Tuesday morning. We saw the David on Sunday, but by the time we got to the Uffizi it was an hour or two from closing and the line was way too long, so we didn't get there. We didn't care that much, but if you do, you might re-plan or just make sure and prioritize your stops. Monday, we walked by the Medici Chapel (also closed, as it turned out, because it was the first Monday of the month), ate at the Mercato Centrale, visited the Duomo, and walked up to Piazzale Michelangelo. There's plenty to keep you occupied for a day even if it's Sunday, but if there are specific things you want to see, definitely check schedules ahead of time.
