But you may encounter the dreaded "Turkish toilet," which is basically a hole in the ground, especially at inexpensive cafés. Also, some of those charge for use of the restroom. They're often down a flight of stairs.

In some older establishments they still have unisex rest rooms - usually with an older woman as "guard". These have several stalls that are used by men or women - that cover you from shoulder to knee - and you just have to hope there isn't a tall man in the one next to you. (In these places I always take the stall on the end and act quickly.)

And in many fast food places (McDonalds, Quick, etc) you might find that the restroom door is locked and there is a keypad. You will need to have your receipt handy, and look at the "combo" - 3 or 4 digits to type into the keypad. They do this so you don't just come in to use their facilities. I live outside Grenoble, and the restaurants in town have these, but out in the 'burbs they do not. : )
