I want to visit five major cities in Italy: Milan, Venice, Florence, Pisa & Rome/Vatican City for two weeks. which city should I fly in first and what's the order of the cities I should put on my itinerary? your help is greatly appreciated!

Fly into Milan and out of Rome to get the best weather.... heading from north to south as the weather gets a little cooler.
I'd do Pisa as a day trip from Florence. Your order is fine.
Are you talking about 12 days out of 14 on the ground with 2 travel days? If so, I might skip Milan and fly into Venice. Then spend 4 nights each in Venice, Florence and Rome.

Either fly into Milan and out of Rome or reverse as those two airports have more non-stop flights from the US and are usually less expensive.
You can visit Pisa by train from Florence (about 1 hour, 6€) so no need to change hotels.
