Passenger Locator Form

I see text saying that covid test bookings should go on this form, but I don't see any place to put these booking numbers. Does anyone know how to do this?

The form I filled in a month ago just had a field to provide the name of the test provider. When initially set up the form was supposed to have a field for a test reference number, with a view to linking the IT to that of the providers. That level of connectivity didn't happen, this information is a month old but as ever the situation changes daily.

Don't hold much hope for help from the call centre number on the form. I called twice and they were just a waste of space and tried every trick to avoid answering questions. Indeed, the whole system including The Home office, consulates and Embassies don't seem to want to give any advice.

The test reference number is to be listed in the "booking reference" field under Day 2 and Day 8 testing questions for those who have not been in a red list country in the previous 10 days. Last travelled end of April and there was a space on the form.

Thanks to everyone who replied. Somehow I saved only a profile and could only select "update your details" or "start a new form." I started a new form and I see that I can only complete it within 48 hours of my arrival. I had hoped that I could fill in the test booking numbers ahead of time and avoid rushing and maybe making mistakes, but that is not possible.

Have your receipt handy for the day 2&8 tests, I was asked for it by Border Control.
