I think your criteria are just impossible with a couple exceptions -- at least for Europe. You cannot get to most of Europe in 6 hrs from Boston. You can hardly get anywhere in Europe in 6 hrs from Boston -- I think Iceland and Shannon, Ireland are about it, London just a bit longer. YOu did not get to Budapest from Boston in 6 hrs, nor Lison.
Some of the other things make it very unlikely, also -- ie, no major city, but you want a direct flight and lots of public transportation.
So, I think Iceland is a fine idea, and I do also think Scotland is a great idea (but you aren't getting there in 6 hrs). Otherwise, maybe the west is a good one -- the Durango Colorado area is beautiful and you have a lot to see and do around there, as well as New Mexico. Of course, the Grand Canyon trip and some others are just outdoors things, so certainly couldn't be for everyone. Durango would have some options, though, more than a rafting trip on the river.
Otherwise, I think you have to change your criteria. How about going to Nicaragua? That's different and there is a lot to see there--it's within 6 hrs by AA through Miami. Or maybe something up in Newfoundland or Nova Scotia. I'd even recommend a more different island in the Caribbean perhaps -- like Guadeloupe. I've been there and there aren't many American tourists. It has a rain forest, volcano, corniche, beaches, etc. I think it's about 6 hrs from East Coast (you have to transfer), and it's not expensive. It's very different from, say, Bahamas or Bermuda. But it doesn't have a real built-up public transportation system nor "night life". Nicaragua probably doesn't that much, either.
YOu just want too many contradictory, impossible things IMO.

Sounds like a great company! I would definitely choose Prague or Barcelona - they meet all your criteria except the 6-hr trip. Both probably require a stop, making the trip ~10 hours at best. Ireland is also wonderful and meets the requirements, but may not be "off the beaten path." What about Costa Rica? I believe American offers something that takes ~6hrs there - but the return is longer. Sedona is only two hours from Grand Canyon and is fabulous - Enchantment Resort is beautiful. Let us know what you decide!
