Click on "Where to Request the Card."
"You can request the Metrebus Card by filling out the form and delivering it, together with a passport photo and a photocopy of a valid identification document, with the original document to show, in an Atac ticket office (with exception of the ticket office "Spagna"). The card is personalized with name, surname, date of birth and passport photo. In case of loss, theft or damage of the card, it is possible to request a duplicate at authorized Atac ticket offices upon payment of € 7.00 and after having filled out the specific form and attached a copy of your identity document; in case of malfunction, the service is free. If the applicant is a minor, it is necessary to attach a copy of the document of a parent/guardian and, in case of first issue, to deliver a printed photograph of the minor."

ATAC is Rome. Not Lazio. The airport is outside of Rome and I can't remember if any of the shops sell ATAC tickets. Maybe one of the book shops.
Do you need the full month? Where are you heading that you need the three zones?
ATAC has various offers.
https://servizi.cotralspa.it/frm_Richiesta_metrebus
That's the Cotral offer. You can order it online but looking at the request it doesn't seem aimed at tourists. The page requires an Italian address.
The CIS is available to tourists

Metrebus is an integrated system... Cotral, Atac, and Trenitalia.
"To facilitate mobility within the Lazio Region, the public transport companies Cotral, Atac and Trenitalia created the Metrebus integrated fare system in 1994 , which allows holders of integrated tickets and passes to travel indifferently on the vehicles of the three companies, within the limits validity of the purchased title. This system is divided into Metrebus Rome , which allows you to travel by public transport in the capital area and Metrebus Lazio for travel in the regional territory. The territory of Lazio is divided into seven concentric zones . Each municipality is part of only one area. The areas are marked by the letters of the ABCDEF alphabet: the letter A corresponds to the area of Rome which in terms of size and complexity is worth two.
"With Metrebus travel tickets it is possible to travel:
on multiple means of transport (Rome and Lazio buses, trams, subways and trains);
on multiple routes
within a zone
between multiple zones excluding special connections.
"The price of the integrated ticket depends only on the areas crossed, not on the number of vehicles used or the length of the routes. The subscription can refer to up to seven zones, but its maximum price will correspond to six zones.
Metrebus Lazio season tickets show the number of zones purchased on the card."

There are Atac and Trenitalia offices at FCO and probably Cotral as well.

I’ll be making day trips out of a Rome apartment for the month of June. A number of the trips are in the Orange zone. I can also use the monthly 3 zone Lazio pass to and from FCO and Roma Trastevere (confirmed with Atac that it is valid).
I’ll be using it in Rome as well, so it’s convenient as well as cost effective. I’ve used it on previous trips, just never had to fill out forms and have a photo available
