I've used several different student tour companies. The trip insurance has never been included, but is offered by the tour company. It was up to the students whether they wanted to spend the extra money. I always advised them not to purchase from the tour company. If they wanted trip insurance they should use a company not associated with the tour company as some have gone bankrupt in the past.
None of the student tour companies I'm familiar with include all meals. Typically it's breakfast and either lunch or dinner, not both. Some companies are very skimpy on meals and the kids end up spending quite a bit extra for food as they are always in a touristy area where prices are high.
