Have been very satisified with these two hotels that we have stayed at over the past years.
HOTEL DE LA VILLE, via Sistina, 79 3 star with 189 rooms located at the top of the Spanish Steps.
MARRIOTT GRAND HOTEL FLORA, via V. Veneto, 191 a 4 star with 151 rooms located at the top end of via Veneto.
Both hotels are in excellant locations
and result in very enjoyable stays.

If you can afford it, the St. Regis Grand, but I've also enjoyed the Aldrovandi Palace, the Lord Byron, and even the avant garde Boscolo Aleph.
A lot depends on what neighborhood you wish to stay in and that is an absolute major bone of contention here.
The one hotel which I think is not 'worth it' is the Hassler but that's just my opinion.
