Possibly rent a car for a few days before the cruise (Carnival), then return car before heading onto the cruise. After the cruise, need a car for a week before heading back to the airport (JAX). Any recommendation? I called Avis and they do not provide pick up service at the cruise terminal. I wonder if there are other car rental companies that would pick up at the cruise terminal without charging a fee.

A regular taxi will cost about the same as that JAX airport shuttle, and a taxi holds up to 3 people. If you're renting a car, that's going to be the cheapest way to and from the airport.
Hertz and Avis both have direct phones in the cruise terminal, but they may charge for a pick-up. I'd first check with them. Then perhaps Enterprise. But I'd just pick up and return the car at the airport pre-cruise and then pay for a single taxi to the JAXport. On the way back, I'd pay for a taxi if necessary to the car-rental company.
