We've used this company, http://www.floridalimo.com/. If just a few people, you can grab a shared ride after you get your bags. $18 a person into Miami and they have a stand right outside baggage claim. We've also used their limo service. No problems.

We just did exactly this in February, flew into FLL and sailed out of MIA. After researching all of our options, we decided that renting a car was the way to go. We used AVIS, and the one word of advice I would give you is to do their online check in. When we got to the airport, the line was a mile long and we just breezed right by everyone, got in our van and left. We returned the van to the Avis port near the MIA Airport, and took their free shuttle to the cruise port. Just beware, if you don't like driving in heavy traffic you will not like this drive. It says it should be about a 30 min drive but with traffic it was closer to an hour, so leave plenty of time, especially on the return.
