I was in Burma last month. I booked with Escapes Unlimited (www.escapesltd.com) and toured Rangoon, Bagan, Mandalay and Inle Lake with just a guide and driver. Escapes uses Indochina Tours out of Bangkok.
The trip was fantastic. Burmese people are the nicest, most hospitable, most welcoming you'll find anywhere in the world. Bagan is as interesting as Angkor Wat but gets 10% of the tourists. Go to Burma! Everyone I met said to send more visitors.

I used, and highly recommend Rubyland Tourism Services - www.rubyland.net.
Although every company I contacted sent me 'tours' and prices, once there I found out prices are all negotioable.
