Phuket and Chiang Mai
My soon-to-be wife and I are in the final planning stages of our Thailand honeymoon. We are so excited and can't wait to leave in 3 weeks! We have our places and dates planned, we are now working on the daily activities. We are spending 5 days in Phuket followed by 4 in Chiang Mai. The hotels were are staying at are the Banyan Tree Phuket and the Chedi in Chiang Mai. So far, in Chiang Mai, we have a day trip to Doi Suthep and the Elephants at work excursion planned, as well as a a cooking class.In Phuket we were thinking about doing a half day cruise, but nothing definitive yet.
Other than that, we are open open to ideas. Any suggestions on restaurants in either place? Or recommendations of activities that you enjoyed doing?
Any help would be greatly appreciated.

One of the best restaurants on Phuket (IMO) is Mom Tri's Boathouse Grill. It's located at the Mom Tri's Boathouse hotel on Kata Beach. Another Mom Tri's restaurant is the Kitchen, located at the Mom Tri's Villa Royale property in between Kata and Kata Noi beaches. The Kitchen has the best ambiance (or it did last time I was there, anyway). Both places have an extensive wine list, if that is of interest. Service is excellent. This would be a special 'treat' and not just a regular evening meal, I would think.

I'm not sure what the half-day cruise in Phuket is, but visiting the island and not taking a tour if Phang Nga Bay (preferably by sea canoe) is something not to be missed, IMO. It's a full day trip.
It sounds like you're hitting the major sights in Chiang Mai. You may want to consider a stroll through the old city and/or down the river.

Doi Ithanon National Park about 40k from CM is definitely worth a visit for a change of pace, either with a rental car (4WD is preferred) or a guided tour. Highest mountain in Thailand, forests,waterfalls, minority tribe villages etc.

Khao Soi is an northern thai speciality noodles & curry with loads of different condiments - chili paste, sugar, pickled cabbage, lime, fish sauce even bananas. It is readily available on the street and in the markets but at "Just Khao Soi" on Thanon Charoenbpratet they have elevated it to an art form. Nice surroundings and great service.

I second Doi Inthanon. It's fantastic! The gardens there are some of the prettiest I've seen anywhere.
And the drive up to it is interesting. My Thai friend and I stopped at a small roadside market where we bought all sorts of really cheap trinkets and bling!
