Nepal visa- get it in US or Nepal if we have a short runway?

I have no idea what you mean by "have a short runway." But it is fast and easy to get a visa on arrival if you travel on a US passport. Fill out the paperwork while you are still on the plane (they hand out the forms) and make sure you have the correct change for the visa.

Does "short runway" presumably mean short lead time?
I just got home to the US from Nepal and can confirm, as Kathie says, that visa on arrival is easy to do. No forms were given out on our flight (Jet Airways 9W from Delhi to Kathmandu). In that case, you just walk into the arrivals area and look for the tables with folks crowded around to find the form you need. Earlier this month, the shortest stay visa for a US citizen was a 15 day stay for USD$25. We brought cash.
Speaking of runways, Tribhuvan International Airport in KTM has only the one runway. International flights get preference over domestic flights.
