When I was in Tokyo, I stayed at the Grand Hyatt....and I thought it was wonderful. Gorgeous modern interiors and wonderful service... http://tokyo.grand.hyatt.com/hyatt/hotels/index.jsp
FWIW...I've stayed at the Pen and Mandarin Oriental in Bangkok (but not in Tokyo, just comparing) and I liked the Grand Hyatt best.

Mandarin or Peninsula are your best bets. They're the newest and therefore the most modern; i.e. newest gizmos and technologies throughout the room. People who aren;t used to Japan may not know what I mean here, but every time I stay in a place that's new/upscale I encounter at least one amazing feature.
Several other properties like Conrad, Ritz, 4 Seasons, Grand & Park Hyatt are also great... but when it comes to Tokyo, new & foreign high-end is usually best, that mixes the best of Japan with global standards.

All of the above mentioned a great hotels in Tokyo and well located in their own rights. I would tend to rank it as the Peninsula, 4 Seasons chinzan so & 4 Seasons Marunouchi, M Oriental, P Hyatt,Ritz Carlton for the 5 stars in Tokyo. I think the G Hyatt is a 4.5 star officially but should be a 5 and is where we are staying this October
KimJapan of the Asia board recently did a review on the Peninsula. See link below

You really can't go wrong with any of these hotels
Aloha!
