We did something very similar to your day 2 with Sgt Kai (actually, on that day we had another of his guides, but we had him when we went to the Elephant Conservation Center the next day). Both days were great. I'd highly recommend him. My trip report's lengthy, but you can scroll down the part on Chiang Mai if you'd like to read a detailed journal of those two days. Here's the link:

The days sound packed. How many people are you...just curious, as I had seen another person post about his pricing and it was much higher than what you have been quoted if I remember correctly.
It's 3 hours drive to the Golden Triangle from Chaing Mai according to the Anantara...meaning 6 hours of Day 3 will be in the car driving. The tour on this day is the typical Golden Triangle tour that I have read reports of as being disappointing and tacky. I would skip Day 3, or replace it with a less tourist trap kind of day.
Just one thing about him, too...I've seen quite a few reports where he was supposed to be the guide himself, but sent someone else instead.
