Hi M,
It's strictly an advertising term intended to elevate any small hotel to an upscale, exquisitely charming, amazingly lovely enterprise, with excellent service, fine food and a "world class" clientel.

Not in a chain
Smaller in size (10-30 rooms)
Faultless decor
Higher level of services
Higher-end price
ie.
Hotel le Vignon, http://www.levignon.com/
Hotel Montalembert

There are many nice true boutique hotels in the intended sense of the word. But I have found lately that many other hotels not really qualified to use the term are hopping on the bandwagon. As they do, the word "boutique" has gained a meaning in my mind of "artificially cute, pretentious, small, overly expensive, and somewhat snooty."

"A reasonably priced, charming and cozy boutique hotel in a pleasant neighborhood, away from the crowds of tourists, conveniently located to all of the major sights", generally means a small hotel, tiny rooms, out in the sticks, 400 m uphill from a bus stop.
Think "6 rms, river view".

There is obviously no uniform "definition" and one example cited here is the idea that the hotel be 10-30 rooms in size.
I', staying in a Paris hotel next week which has a total of 68 rooms and it describes itself as a "boutique hotel."
Subjectivity always nakes things more interesting such as hotel room descriptions using the terms "large" or "huge" or "small" or "dirty" etc., etc.
I suspect from your screenname one thing that DOESN'T qualify is mold on the walls of the shower.

Intrepid1-
You are absolutely right about the bathroom phobia I have! The bags have barely been set down before I have a look at the bathroom! The hotel that inspired my username will be remembered forever in horror!
