I used the NY Airport Service for that that a few years ago - see http://www.nyairportservice.com/ - it was reasonably fast and inexpensive, looks like you can save if u purchase online.

Carey bus company http://www.groundnet.com/carey/airport.html has shuttle service from LGA to JFK. Look for the curbside signs at the arrivals area.
If you get in before rush hour starts (~3:30) and there's no Mets game, the ride should take about 30-40 minutes.
