> I don't remember the name of the company
When you book a car through Hotwire in Lihue, Kauai; you will get a car either from Hertz, Enterprise, Alamo / National, Dollar, Thrifty, or Sixt. All except the last are top-name brands, so I'll assume it's Sixt that we should watch out for.
Interestingly, I don't see any guarantee, either at Hotwire or at Sixt, that you will get a non-smoking car if your use their services. I suppose you could ASK for this, but it's like asking for non-smoking room at a hotel -- it's only a REQUEST.
If this a critical issue, check out

Sometimes the savings is worth the $55 cost of joining Costco [which can be done online], even if you don't go a store.
Makes sense. It's not going to help for the Alaska trip, but I'll keep Costco in mind for the future. Thanks.
