I'd recommend reposting this in the Caribbean travel forum. Just change the forum name. Tag it Puerto Rico. There won't be a tag for Colombia in the Caribbean forum since it's in South America, so you could also post separately in that forum.
I think the main difference between the two is that PR is in the US and Colombia is not. And the beaches in Cartagena are not really near the historic part of the city, so you have to decide what kind of vacation you want to have. That's less of a concern in Puerto Rico since there are only a handful of hotels in Old San Juan.
Cartagena is on our list of places to go in 2014, so you may wish to read up on it and see what you think. One of our editors just went and had a lovely time in Cartagena. I've been to Puerto Rico several times and like it, but I've never been to Cartagena.
I think the two destinations are pretty much equidistant to Miami, but it may be cheaper to get to Puerto Rico.
