VG...I wonder if you are referring to the same gift shop I went to. Had to kill some time waiting for our plane and since there is NOTHING to do at the "airport", I walked around the corner to a little gift shop that was quite interesting.
BAJJ...I agree about no car. The taxis we used in Arenal were reasonable and it was worth not dealing with a car. We basically only needed transportation for dinner on two occasions since one day was a full day's outing to Cano Negro. I was wondering which driver you are using for your trip from Arenal to SJO.
lavryder...somewhere in OSA would definitely be my choice. I didn't visit Manuel Antonio and may people like it a lot. But I get the feeling that it would be too touristy for me. BDC was wonderful and has great wildlife viewing. It is a little pricey, but considering that all your meals are included, it's not a bad deal...especially with 20% off. However, I was really tempted and checked out the deal, and it is actually a 5th night free with 4 paid nights. Based on your trip duration, it sounds like you might be able to swing that.
I would also really like to visit Drake Bay, but that will be a future trip. If you like fishing and snorkeling/scuba diving, it might be an option too!

Deb - Yes, it is actually a 5th night free. I should have been more clear. We're using Tucan Limo (Roadadvisor) for two transfers - from SJO to Monteverde and then Arenal to the NatureAir terminal.
