Flying in and out of Rome, the simplest thing to do is to pick up and drop off the car at the Fiumicino airport, where I assume you will be going. I'm not familiar with the Villa S.Pio Aventino, so I don't know if that is a Rome hotel or a Tuscany hotel. If you aren't staying in Rome after you land, certainly rent the car and head for Tuscany. If you are staying at a hotel in Rome when you arrive, then rent the car at a downtown location in Rome only when you are ready to leave for Tuscany. And if you are staying in Rome when you return there, turn in the car at the airport, or in Orvieto and catch a train to Rome.

If you are staying in Rome for a few days, you could do what we did and take a train out of Rome for a couple of stops and rent the car just outside of Rome (rather than hassling with downtown traffic!) As I remember it was the same cost or even a little cheaper... sorry I don't remember who we rented it from cuz it was about five years ago, but it was a great way to do it - If it wasn't Hertz or Avis it probably was Europcar.

Your hotel is located in the Aventine near the Circus Maximus. I think it would be easiest to take a taxi to the termini train station and pick up and return your car from there. I personally think the 'difficulty' of the drive in and out of the city is much ado about nothing.
