There are no ferries between Amalfi and Naples.
Are you going between April and end October? In that case, a ferry from Amalfi to Salerno and then a train. You could get one that goes all the way to Milan without a change. There are a few of those per day. Check the Trenitalia site closer to your travel dates.

If you are travelling out of season, a bus is your only road method out to the train - Salerno, again, is closer to Amalfi.
Otherwise, get a car service to take you to Salerno for the train connection - cost would be about 80-100 euro.

Thank you, didn't know we could go all the way from Salerno to Milan It must take forever. I will check it out.
I would appreciate your insight on this plan. We are flying into Milan(got cheap fares) but want to spend 1 day in Rome, can't decide whether to fly or take the train to Rome. Then from Rome take the Train to Salerno and the Hydrofoil to Amalfi where will are staying for 5 nights. Then back Milan for 1 night and flying back home.
BTW, we are going in May.
Thank you, again!

Depends what time you land in Milan - are you overnighting there? Can you get an onward flight connection to Rome and then clear immigration in Rome? That would save time.
If you have to clear immigration into the Schengen zone in Milan, retrieve your luggage and then take another domestic flight - with all its processing, checking in, security measures and so on - a train to Rome could be more efficient timewise. Then again, if you land after 6pm, by the time you collect your luggage and get to the station and take the 3 hour trip to Rome it may be past midnight.
Keep an eye on the ferry timetables from Salerno - they are uploaded close to the season. At the moment it only shows 2015 times, with the last ferry leaving Salerno for Amalfi after 2.00 so in order for your plan to work, you need to be in Salerno before then. It is about a 15 minute walk from the train station to the ferry dock.

It takes 5.5 hours to get from Salerno to Milan. The trains are very comfortable, they have wifi, you can get first class tickets that include a meal/snack and electric outlets at the seat. I've only taken first class once - from Venice to Naples - and frankly I didn't find much of a difference except for the food service - which wasn't that great.
If you book through Trenitalia in advance you can get very low fares. They start selling the low fares four months in advance so check closer to your travel times.
