If you are taking a cruise, then the train is cheap and easy. It is basically a commuter run. Roughly three trains an hour ranging from 45 mins to 1-15 and 5 or 6 E to 12E depending on train. From the train station it is a several block walk along the beach to the port entrance - maybe a half mile. Probably can see your cruise ship when you exist the station. From the port entrance a shuttle bus will take you to your ship. If returning to Rome be sure to buy return tickets because the little ticket office in Civit... is a zoo with loooooong lines when the ships return to Civit.. You can use taxis or shuttle but the price will be closer to 150E.

There is a company called Romecabs who will pick you up at the airport and take you directly to Citavcchia. They are very reliable. You can find them at: http://www.romecabs.com.

There is a company called Fattori Car Service.
They have a shuttle service from Rome city or Airports to Civitavecchia at very reasonable prices.
Very reliable and quality service.

Train from FCO to Rome Termini then transfer to train to Civitaveccia.
There are some buses from the same ferry/cruise taking passenger to and from the ships, we were taken in a bus from the ship to the train station. There was a machine to buy tickets so there is no need to be in line, at least we didn't, office was closed that day.
