Transfer from Civitavecchia to FCO
Can anyone suggest a transfer company from the port to Rome airport FCO? There will be 2 of us and the cruise line wants $129/pp. I do have an early flight ( 11am) so would I be better off with the cruise line transfer?

A taxi is about 120€.
A train from Civitavecchia to Rome Trastevere station takes anywhere from 49-65 minutes (depending on train) and cost is 4.60€. From Rome Trastevere (you must change trains here), the FR1 train departs every 15 minutes for the airport (8€)and travel time is 28 minutes. It would depend whether you get off the ship early enough to make your flight. If it's an international flight, it's a trek/process just getting from the terminal where you must then catch a shuttle bus to passport/ticketing/baggage check and security. After checking in etc., you then catch another shuttle to the terminal. You need to allow plenty of time to do all of that.
Limo service is 110€ (up the 3 people including luggage).

Unless they let you off the ship before 07:00, you aren't going to make it.
It's an hour to make the drive.
You have to arrive 2:30 hr before departure.
Figure 1/2 hr for miscellany.

The ship will allow you to disembark early in order to make a flight but NOT before it has been cleared by the local authorities.
I would check with the airline to see if there is a deadline for checking LUGGAGE in. It is not going to take you three hours to clear immigration/customs/security at the airport

PS,
We used http://www.airportshuttle.it/
They got us from Rome to Civitavecchia on time and without driving at 100 mph.
A limo for 2 is about 140E.

I just flew home from FCO on an 11am Alitalia flight last week. Alitalia left from Terminal 3 for US destinations. It took me a total of 10 minutes to pass through security at T3, so I had plenty of time to kill at the gate.
If your flight is a US flight and bound for a US destination then it may leave from T5, in that case you may need a little bit more time. Otherwise I'd say to arrive between 2~2.5 hr ahead of departure is plenty.
Also, the cruise ship I was on docked at Civitavecchia port at 5am. The cruise line knows how to schedule arrival time so as to make it easy for its customers.
