If you have only two or three days: Milan, which has one of the most beautiful cathedrals in the world. If you have a week or more: Rome. If time and money are not obstacles: Why not both.

I've been to both and Rome is the hands down winner. The highlights of Milan are the cathedral and daVinci's Last Supper fresco. On the other hand Rome has: St Peter's Basilica (Michaelangelo's Pieta), Vatican Museums (Sistine Chapel, Raphael Rooms, etc), Catel Saint Angelo, Roman Forum, Colosseum, Pantheon, Capitoline, Circus Maximus, Teatro Marcelus, Piazza Navonna, Trevi Fountain, Spanish Steps, and on and on.
