Do a search above for hotels near Rome airport-there are a couple of good suggestions.
Looking at my notes for our last trip to Rome, our hotel was near Piazza Navona and a cab picked up at 6:50 and got us to airport by 7:15 for our 8:55 flight. That seems too fast so I'm not sure now. Google says 47 minutes.
The mosaics in Ravenna are pretty spectacular.

Do you want the stress of ensuring that you arrive at Fiumicino airport by 8:00 a.m. from a town over an hour away?
I think your plan to do simply use Rome as a departure airport is good, but would suggest that you find a nice airport hotel and simply plan to check in late after a full day of touring on your way down from Tuscany. If your plans or desires change at all, you have the option of checking in a little earlier and having an exciting, brief foray into Rome for dinner and walk.
The owns you are suggesting for your Tuscany are good choices for your husband's back condition. Siena, would hold the most "attractions", and offer the best opportunities for day trip travel because of it's location and size since it is a moderately sized city. The others are all small towns with outlying or infrequent, at best, rail service. Volterra is probably the farthest "out of the way", and if you are going to be using the train, then the options for day trips will be a little more limited. You should consider looking into buses for day trips, too.
It would be great if you could work Ravenna or Ferrara in as s stopover on your way to Florence. Storing your luggage easily is the biggest issue, but I would investigate options. That will save you almost an entire day.
I like the idea of 4 days Venice, for "recovery" and you could day-trip to one of the other islands, or Padua, Verona, etc., and then finding a way to take in Ravenna or Ferrara on the travel day to Florence. The 3-4 days in Florence, and 3-4 in Sienna/Tuscany before late arrival at a Rome airport hotel.
