If you have 5 train travel days within Germany, and you need to buy a 6-day 2nd class railpass with extension for a trip to Venice, the price of the extra railpass day is €23 more. That's €72 - US $98 - total with the extension>
It appears you this time are wrong in saying you need to buy one more day on your pass - or is bahn.de wrong? According to them you only need add on $45 and not buy an extra day - please tell me where I am wrong as I sure I am and you are right!

"...you only need add on $45 and not buy an extra day..."
(It's NOT $45 for the extension - it's $72 (€49.) That info is on the DB page you just linked to in your post above.)

It's fairly straightforward, PalenQ.
Let's say you are already making 5 trips within Germany using your railpass, and you want to add a trip to Venice; well, your 5 travel days are all used up already. You need permission to use the pass outside Germany and get to Venice (extension) but that's not enough. You need to travel to Venice, and you need a railpass day for that, so you need a 6th day on your pass.
If you are already making 6 trips within Germany using your railpass, and you want to add a trip to Venice, well, your 6 travel days are all used up already. You need a 7th day on your pass.
If you are RETURNING to Germany after your extension trip to Venice (or wherever) and have already made 6 trips within Germany using your railpass, and one trip to Venice on a 7th railpass day, well, your 7 travel days are all used up already. You used the 7th day on your pass to reach Venice. And you need an 8th railpass day on your pass to return from Venice to Germany. You paid your €49 for the privilege of using the German railpass outside Germany (extension) and there's no additional €49 charge to use the extension when you return to Germany. But you STILL have to have a 7th and an 8th valid railpass day.
The extension allows you to travel outside of Germany to Venice (or wherever) on one of the railpass days you purchased; it does NOT add a railpass day to your pass. (It's a good way for DB to boost sales, isn't it?)
Rick Steves' site says it like this:
"German Rail Pass Extension: International trips must take place on the same dates that you use the German Pass."
The DB page says it like this in the FAQs:
"My German Rail Pass is valid for 5 travel days within 1 month, how often can I use the German Rail Pass Extension coupon?"
"The German Rail Pass Extension is valid on all purchased travel days, in this case on 5 days."
The Raileurope site says this:
" German Rail Pass Extension is only valid in conjunction with a German Rail Pass.
German Rail Pass Extension does not alter the number of travel days on the German Rail Pass."
The only time you don't need to buy an extra railpass day (or two) for an additional journey to Venice is when railpass days aren't even a question (like with the continuous pass, like the 15 travel days within 15 days Eurailpass.)
"It appears you this time are wrong... please tell me where I am wrong as I sure I am and you are right!"
It's hard to tell exactly what you think, PQ. Why don't you just look at the examples I gave and the evidence in this post, and go to the other sites if you want, and then sort out for yourself whether I'm right or wrong.
I contribute to these boards pretty often and do make mistakes at time, but I think I typically take full credit for these mistakes when they happen.

Well I am sure you are right, having done all that - a quick glance like I took at the link I gave could be construed various ways but I think Russ has it right, as usual.
