If you are there any other day the best place will be where the offer is. These do change but sometimes you can get a new phone, SIM and call time all for about 20 to 30 €.
The main suppliers of Services are Movistar, Orange, Vodafone, but there are others too.

My daughter is in Toledo and needs phone for getting in touch with other students in Spain. She has iphone which she'll use only for wireless/skype .She has GSM phone which she can hopefully unlock(has the code). So she needs the cell for in Spain use. Is it cheaper to buy just SIM or buy phone with SIM. Also do you buy from mobile stores, kiosks or department stores.
